On Lok SeniorHealth, a Program of All-inclusive Care for the Elderly (PACE), provides long-term care for eligible seniors living in San Francisco and Fremont, Calif. The program offers full medical care and support services with the goal to help seniors live at home and in the community for as long as possible. This comprehensive health plan includes medical care, as well as prescription drugs, adult day health services, home care, transportation and other support services. Languages spoken include Cantonese, English, Italian, Japanese, Korean, Mandarin, Russian, Spanish, Tagalog, Toishanese, Fukanese and Vietnamese.
